Dr. Jolene Miller is a Certified Pelvic Floor Physical Therapist. She received extensive training from Herman and Wallace Pelvic Rehabilitation Institute. This certification allows her to treat a wide range of urinary, bowel and sexual dysfunctions.
She also specializes in; Orthopedic, Sports Injuries, Pediatrics and Neurological Injuries. Her patients age range from 4 months to 98 years of age.
She believes that with one-on-one treatment with our extensively trained and focused therapists every individual can be assisted to find the recipe for success and a life without pain and dysfunction.
When she is not working she enjoys spending time with her family and friends!

Dr. Noah Miller, PT, DPT
Owner, Physical Therapist
Dr. Noah Miller is specially certified by The American Institute of Balance. This Institute is lead by the world-renowned Dr. Richard E. Gains. They are the leaders in the most effective examination techniques to treat balance dysfunctions like BPPV.
Dr. Noah believes in staying current with the newest and most advanced physical therapy techniques proven to provide optimal outcomes. He also prefers to use dry needling as a method to treat soft tissue injuries (muscle, nerve, tendon) with dry needling as it’s a conservative way to rid spasms and trigger points without medication.
Dr. Noah has experience treating patients in various PT settings ranging from acute, sub-acute, and outpatient clinical settings. He believes that manual/hands-on PT coupled with a specialized care plan is the key element in rehabilitation.
When not in the office, Dr. Noah enjoys spending time with family and friends. He is very excited to be a part of the Garden State Physical Therapy Group.

Dr. Bryan Connaughton, PT, DPT
Physical Therapist
Dr. Bryan was raised in Central, NJ now residing in Eatontown, NJ. Playing baseball and football growing up, he began his intrigue and passion for the human body. While attending Notre Dame Highschool in Lawrenceville, NJ, this spark was further enhanced after completing a Sports Medicine class introducing him to anatomy, sports related injuries, and rehabilitation principles.
He went on to pursue pre-professional studies earning his Bachelor of Science in Biology from Iona College in New Rochelle, NY in 2013. Following, he went on to work as an Rehabilitation Aide in a acute care hospital to begin to understand an interdisciplinary care, provided at GSPT. He was then accepted into the inaugural DPT class of Kean University in Union, NJ. During his clinical rotations, he was exposed to various aspects of the profession and gathered invaluable experience in vestibular and concussion rehabilitation. He would go on to receive his Doctor of Physical Therapy degree in 2019.
Dr. Bryan began his career traveling the country, acquiring licenses and working as a Doctor of Physical Therapy in Indiana, Oklahoma, and Maine. During this time, he completed contracts in a multitude of settings including acute, sub-acute, skilled nursing, and outpatient. Ultimately, he returned to NJ to provide care to those in need in their homes to further his patient-centered approach to care . This has brought a unique perspective and skill set to his practice, understanding various approaches to care that he has brought to his new home at Garden State Physical Therapy.
He is dedicated to a lifetime of learning with continuing education focused on understanding Postural Restoration Institute (PRI) principles that include asymmetrical human patterns and the integral role of breath/diaphragm. He has also completed course regarding concussions and vestibular rehabilitation. While not practicing, Dr. Bryan enjoys staying active while hiking, golfing, skiing, and keeping his dog, Luna, throughly entertained. In addition, he thrives playing fantasy football, smoking meats, manicuring his lawn, and being a self-proclaimed expert at backyard games.

Dr. Chelsea Lane, PT, DPT
Physical Therapist
Dr. Chelsea Lane was born and raised in Staten Island, New York. She attended SUNY Albany, during which time she studied abroad in Europe, and graduated with her Bachelor of Arts degree in Psychology and minored in Biology. For graduate school, she attended Kean University for her Master of Science in Exercise Science, where she performed research for Chemotherapy Induced Peripheral Neuropathy. Dr. Lane received her Doctorate of Physical Therapy from Kean University, and has worked in areas such as sports and orthopedics, pediatrics, hippotherapy, acute/trauma, skilled nursing facility, and school-based settings.
Dr. Lane has practiced in various settings during her time as a Travel Physical Therapist. During her hospital work, she was placed on the total joint unit, allowing her to work alongside surgeons for patients pre and post surgery. She traveled the country working in different states including Indiana, Oklahoma, and Maine – picking up her pup Luna along the way! Dr. Lane also has worked heavily with the geriatric community to regain their independence in their own home, as a Home Care Physical Therapist throughout New Jersey. Her continuing education has sparked her interest in Postural Restoration, focusing on the body’s natural asymmetries in order to restore balance, decrease pain, and improve the quality of life of her patients.
When she is not practicing, Dr. Lane enjoys traveling, hiking, camping, and spending time with her family and friends.

Dr. John Baylon, PT, DPT
Physical Therapist
John completed his Bachelor’s in Physical Therapy at Emilio Aguinaldo College, Manila, Philippines and went on to earn his Doctorate in Physical Therapy at University of Montana, Missoula. He has completed Clinical Affiliations in major hospitals and rehab facilities in Manila and California. He passed the National PT Licensure Board Examinations in both countries before settling in New Jersey where he worked in a variety of settings including in-patient/out-patient hospital, sub-acute rehabilitation, home health care, and pediatrics with the NJ Early Intervention System.
For nearly three decades clinical experience, he has particular interest and spent most of his career in orthopedic out-patient cases and completed various coursework and certifications through the years including Orthopedic Manual Therapy, McKenzie, Balance-Vestibular Rehabilitation, Comprehensive Sports Rehabilitation, IWA Strength Trainer Certification, Society for Golf Medicine and Performance, to name a few.
John’s approach to treatment is a skilled hands-on therapy that utilizes a variety of evidence-based physical therapy modalities including exercises and manual techniques.

Dr. Timofey Rubanyuk, PT, DPT
Physical Therapist
Timofey Rubanyuk DPT completed his undergraduate studies at the University of Connecticut and earned his Doctorate in Physical Therapy from Touro College in Manhattan, NY in 2016. Tim has extensive experience working with various patient populations, including geriatrics, pediatrics, post-operative care, and sports injury.
Pain, disability, injury and performance are all influenced by a variety of factors in an individual’s life. Tim works togethers with his patients to find and address the multitude of these contributing factors in a comprehensive and inclusive manner. Tim believes that best results come from forming an effective partnership with the patient, guiding them through their recovery and assisting them in becoming well-informed and highly motivated actors playing an integral role in their own rehabilitation process.
When not working with patients, Tim dedicates the majority of his time to his family and children. Tim is also an active Brazilian Jiu-Jitsu competitor as well as an Ice Hockey Official.

Dr. Justin Mairs, DC
Owner, Chiropractic Physician
Dr. Justin Mairs is a Board Certified Chiropractic Physician in the State of New Jersey and New York.
Dr. Justin Mairs began his path in healthcare by graduating from Kean University in 2015 with a Bachelors of Arts in Biology. Wanting to further his knowledge of healthcare and his passion to help others, he decided to pursue a career in chiropractic.
Dr. Mairs graduated from the University of Bridgeport School of Chiropractic in 2020. After working in multiple clinical settings, he has had the opportunity to work with the general public and athletes. He takes great pride in helping patients who are in need of becoming free of pain and have goals of restoring any functional limitation to return to their activities of daily living.
Some of his hobbies when he’s not in the office includes working out at the gym, outdoor activities, and cooking. He enjoys learning new information on the latest research in regards to patient care so he can continue to deliver the highest quality of care to his patients.

Dr. Sue “Soula” Disakias, DAc.
Acupuncturist
Dr. Aspasia Disakias, otherwise known as ‘Sue’ or ‘Soula’, is a Licensed Acupuncturist who graduated from Pacific College of Oriental Medicine (New York) with a Master’s and a Doctorate in Acupuncture in 2019/2020. The majority of her education was completed at Tri-State College of Acupuncture in New York where she learned various styles of acupuncture that adapt to each patient’s needs including Kiiko Matsumoto (KM) Japanese Style, Acupuncture Physical Medicine (APM), and Traditional Chinese Medicine (TCM).
From a young child, Sue’s been a lover of nature, science, and the arts; acupuncture served as the perfect balance that fed her passion to help people. She has been interested in holistic healing since she tore her ACL playing lacrosse in high school. She underwent Physical Therapy and experienced the body’s astounding self-healing power. She’s also had severe acne skin concerns in the past that was healed through holistic means and a change of lifestyle rather than the traditional Western approach.
Sue enjoys enhancing the sensory and spiritual experience by incorporating high quality essential oils, CBD topicals, and healing crystals throughout her treatments. Her strengths as a practitioner lies in her intuition, attention to detail, consistency towards finding answers, and her listening ear. She offers her patients a well-rounded, consistent acupuncture experience that they come back for again and again. She loves to incorporate mindfulness and self-awareness techniques that allow for her patients to live their lives differently and more positively. Lastly, in her emphasized approach towards pain conditions, myofascial trigger point release is her best friend as it can ensure an immediate result in reducing pain and discomfort.
Sue is willing to show her patients the way of Eastern Medicine and how it can be an effective complementary method towards healing mental, emotional, and physical ailments.

Dr. Moti Yahav, DAM, L.Ac., LMT, CPT
Licensed Acupuncturist & Board-certified Herbalist (Diplomate)
Doctor Moti Yahav is a New York & New Jersey state licensed acupuncturist & board-certified herbalist (Diplomate). He specializes in treating a variety of conditions such as pain-management, rehabilitation, emotional conditions (stress, anxiety, depression, grief), sleep issues, stomach & digestive conditions, vertigo, ear/nose/throat, headaches, women’s health (fertility) and weight management. He utilizes a combination of Western & Eastern Nutrition, Acupuncture, Herbology, Tui-Na/Acupressure, Cupping, E-stim, Gua Sha, stretching and moreas needed for treatment.
Dr. Yahav graduated from Pacific College of Oriental Medicine with a Doctorate in Acupuncture and Chinese Medicine and a Master’s of Science in Traditional Oriental Medicine. During his studies, he held an apprenticeship for the renowned Master Fukui Yang and has taught and tutored Massage Therapy and Traditional Chinese Medicine (Biology,Acupuncture, Herbology) to students both privately and at Pacific College of Health and Science.
Additionally, Dr. Yahav is a New York state licensed Massage Therapist, Nationally certified personal trainer, and a licensed youth-soccer coach. His Bachelor’s degree was in exercise science, and has worked one-on-one and in group settings personal training, teaching principles of physics and kinesiology (movement therapy). He has a passion for his family & friends, nature, animals, sports, martial arts, science, learning, laughter and helping others.

Bo Jung Choi, L.Ac, Dipl. Ac., LMT, CPT

Bo Jung Choi, L.Ac, Dipl. Ac., is a licensed acupuncturist in the state of New Jersey and New York, who is nationally certified by the NCCAOM. After completing his Bachelor’s Degree in Biological Sciences at Binghamton University, Bo continued on to graduate with his Master’s Degree in Acupuncture at Tri-State College of Acupuncture in New York City. Bo practices three distinctively unique styles of acupuncture: Traditional Chinese Medicine (TCM) style, which focuses on finding and balancing the underlying root cause of an ailment, Acupuncture Physical Medicine (APM) style, which targets and relieves a specific muscle trigger point/s, and Kiiko Matsumoto Japanese Acupuncture (KM) style, which painlessly and precisely reduces stress on the body and regulates bodily functions. By utilizing these multifaceted styles of acupuncture and other holistic modalities, Bo aims to provide exceptional patient-specific treatments to effectively assist each patient reach their wellness goals. Brooke Newman, OTR/L, MOT
Occupational Therapist
Brooke graduated from Quinnipiac University. Her experience includes working in outpatient as well in the acute hospital setting. She has worked with both adults and children. She is full of energy and brings it to all of her patients.
Brooke has wanted to be an Occupational Therapist for as long as she could remember. She loves to engage with her patients and listens to them to give them the fullest experience. Brooke continues to grow and learn, as well as stay up to date on all of the latest OT trends and techniques
Outside of work you can find brooke working out and kicking around a soccer ball with friends!

Gabrielle Yosuico, OTR/L, MOT
Occupational Therapist
Gabby is an occupational therapist. She believes in using a holistic and individualized treatment approach to help individuals live meaningful lives despite the disability or injury that they have.
Gabby earned her Bachelor of Health Science Studies and her Master of Occupational Therapy from Quinnipiac University. She has experience working with adults with conditions including chronic/acute pain, orthopedic surgeries, cardiopulmonary, neurodegenerative, SCI/CVA/TBI and multi-trauma regaining their strength and independence. She also has experience working with pediatrics with sensory processing disorder, emotional regulation difficulties, visual motor deficits, decreased fine motor and gross motor coordination in an outpatient setting. She loves working with patients of all ages and all backgrounds reach their functional goals.
Gabby is also a certified yoga instructor. She is E-RYT 200 certified by the Yoga Alliance meaning she successfully completed a 200-hour yoga teacher training. She loves to incorporate mindful movement and yoga asanas (postures) into her treatment.

Dr. Dorothy Ladyzhensky
Occupational Therapist, OTD
Dr. Ladyzhensky received her degree from the University of St. Augustine for Health Sciences. She has been a guest speaker at international conferences on the topic of occupational therapy in chronic pain management, and has presented research on improving the quality of life for individuals who struggle with Fibromyalgia (FM).
Dr. Ladyzhensky utilizes a collaborative approach when tailoring treatment plans for all of her patients. She believes optimal care is achieved through holistic and individualized intervention.

Dr. Kenneth T. Cieslak, DC, LAT, ATC, CSCS
Chiropractic Physician
Dr. Cieslak is excited to become a part of Garden State Therapy Group. He completed his undergraduate degree at Kean University, majoring in athletic training, and his Doctor of Chiropractic degree in 1995, graduating from Life University as valedictorian. He has been a Certified Strength & Conditioning Specialist since 1989. Recently, he completed the Primary Spine Practitioner course, a multi-disciplinary post-graduate program through the University of Pittsburgh School of Health and Rehabilitation Sciences. He is licensed in New Jersey as both a Chiropractor and an Athletic Trainer. From 1997- 2022, he was employed at Teaneck High School, serving as the coordinator of athletic injury care/athletic trainer, in addition to having a practice of chiropractic in Bogota, NJ. He was also a preceptor for the Montclair State University athletic training education program. He currently serves as the President-Elect of the Athletic Trainer’s Society of New Jersey. Ken believes in applying an evidence-based approach to patient care, and frequently co-manages patients with other practitioners. He has additional post-graduate training in a variety of systems, including: Graston Technique, kinesio-taping, functional movement screen, selective functional movement assessment, and McKenzie MDT. He lectures on a variety of topics, including over twenty presentations at the national level, for organizations like the National Athletic Trainer’s Association, and the National Strength and Conditioning Association, and multiple regional and state associations as well. Ken has been married to his wife Eileen, a special education teacher, for 31 years. His family includes his daughter, Erin who is a physician assistant in orthopedics and her husband Nick who is a physical therapy assistant as well as his son Tim, who is a lieutenant in the United States Army currently stationed in Texas.

Nicole Nugent, OTR/L, MSOT
Occupational Therapist
Nicole graduated from Towson University with a masters degree in occupational therapy and a minor in applied adults with disabilities studies. Her experience includes working in inpatient rehab, pediatric outpatient settings, as well as in the school system. She has demonstrated focus in post-surgical care, sensory integration, and fine motor skills necessary in everyday life. Whether working with adults or children, Nicole works to bring fun and games to all her patients!
Nicole believes that everyone deserves to live a life that is fulfilling to them and is eager to help make that possible!
